Center for Life Management (CLM) provides the most comprehensive array of mental health services for children and families in our region. Our multidisciplinary team serves youth ages 3–21 who are experiencing mental illness or serious emotional challenges. We are seeking a Children’s Assertive Community Treatment (ACT) Community Case Manager to join our team and support children and families in achieving stability, growth, and meaningful progress toward wellness.

This is a full-time, 40 hour per week position.

The ACT model is an evidence-based practice that provides intensive, highly integrated, community-based mental health services. ACT services reduce crises, unnecessary hospitalization, law enforcement involvement, juvenile justice system penetration, and out-of-home placements. This role plays a key part in supporting at-risk children, helping them build skills, reduce symptoms, and enhance functioning at home, school, and the community.

As a Children's ACT Community Case Manager, you will:

  • Conduct individual assessments and develop treatment plans with clear goals, interventions, and desired outcomes.
  • Support clients in obtaining and maintaining medical coverage for consistent access to care.
  • Provide outreach, advocacy, resource identification, linkage, and monitoring of services.
  • Deliver counseling and Functional Support Services (FSS) in home, school, and community settings.
  • Provide Therapeutic Behavioral Services (TBSI) to teach and reinforce coping strategies.
  • Offer Family Training and Counseling (FTC) to strengthen support systems and promote resilience.
  • Provide Crisis Intervention (CI) when children experience acute exacerbation of symptoms.
  • Collaborate closely with ACT team members, schools, medical providers, and natural supports to ensure wrap‑around care.
  • Maintain timely and accurate documentation for all services provided.
  • Meet required productivity expectations (16 case management contacts per month, 18 hours of FSS weekly; eligible for incentive pay when exceeding KPI expectations at a rate of $40/hour).
  • Participate in committees as assigned and maintain adherence to confidentiality, professional standards, and agency policies.
  • Uphold all licensing, accreditation, and certification requirements.

The Center for Life Management has partnered with New England College to offer a Master’s Degree! This is an exciting opportunity for CLM staff to earn a Master of Science in Clinical Mental Health Counseling (MS) degree in 3 years.

Who’s Eligible and What’s

Required:

Must be a regular, full-time CLM employee in good standing for 9 months or more.

Must hold a Bachelor’s Degree.

Must agree to a five (5) year commitment of full-time employment in good standing with the Center for Life Management (beginning from start date/year of Professional Development Program participation).

Participants must maintain a B average to continue participation in the program.

* Cost of tuition is covered by Center for Life Management

* Employee is responsible for applicable taxable portion of tuition remission

Requirements Qualifications
  • Bachelor’s degree preferred;
    Associate’s degree with at least two years of related direct service experience may be considered.
  • Ability to exercise sound clinical judgment in routine and crisis situations.
  • Strong ability to engage children through interactive play and therapeutic techniques.
  • Proficiency with Microsoft Office applications and ability to navigate an electronic health record.
  • Strong written and verbal communication skills with the ability to document efficiently.
  • Capable of independent work while being a collaborative team player.
  • Demonstrated ability to learn and apply new clinical skills as needed.
  • Excellent organizational and multitasking abilities.
