Londonderry office, located in Londonderry, NH. Services adult and geriatric patients on a per‑visit basis in homes throughout Salem communities.

• Perform outpatient therapy evaluations to develop a plan of care for patients in designated senior living communities or group home settings for adults with intellectual disabilities.
• Operate at the top of licensure utilizing current research‑based approaches to maximize functional independence.
• Collaborate with home health therapy partners to transition care from home health to outpatient services, achieving client‑centered goals.
• Educate clients, families, and community caregivers on care plans to foster a team approach to maximizing functional independence.
• Perform evaluations, treatments, and discharge planning using the outpatient EMR platform to document individualized treatment approaches.

• Current New Hampshire Occupational Therapist license.
• Minimum one year of recent work experience as an Occupational Therapist.
• Graduated from an OT program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
• Prior home care experience preferred but not required.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem‑solving skills.
• Solid computer skills; prior EMR experience preferred.
