An EPDH in a community outreach setting working with a Medicaid population is a primary care provider. They work independently in multiple community locations, using high‑level decision‑making skills to complete all hygiene services, intra‑oral photos, x‑rays, assessments, and preventive treatment. Using state‑of‑the‑art tele‑health technology, the hygienist communicates and collaborates with the dentist. The tele‑dentist completes a chart review, diagnosis, and treatment plan for restorative needs.

The patient is scheduled for restorative care with a local provider. The EPDH may prescribe antibiotics when necessary, and with guidance from a collaborative dentist, stabilizes a patient until they can be seen by a dentist.
