• Assist patients with navigating the Department of Children and Families (DCF) process.
• Collaborate with interdisciplinary teams for patient care.
• Coordinate services to home care and equipment following discharge.
• Document pertinent information to facilitate care coordination.
• Participate in care conference activities.
• Refer patients to rehabilitation and long‑term care facilities, and to other disciplines/resources.
• Provide individual counseling, marriage counseling, and parental counseling.
• Assist with resources such as AFDC (Aid to Families with Dependent Children), Food stamps, Medicaid, Medicare, Medication Assistance, and TANF.
• Advise on adult protective services, child protective services, emergency detention, guardianship, power of attorney, adoption/foster care, and drug/alcohol treatment options.
• Manage involuntary holds (5150) and related procedures.
• Participate in development of treatment plans.
• Perform biopsychosocial assessments in outpatient clinics, physician offices/private practices, and psychiatric hospitals.

• BLS certification
• Licensure:
  Licensed Master Social Worker (LMSW)
• Experience with EPIC charting system
• Experience with short‑term acute care settings
• Ability to work in teams and handle patient ratios (1:40)
