Bridgeway Community Health

is currently seeking a qualified Care Coordinator for our growing BCC team!

Primary Purpose

Care Coordinator builds an alliance with clients by providing education, and motivation to support them in making positive changes in their lives and reducing the negative consequences and barriers associated with substance use. The Care Coordinator provides client-centered care utilizing evidence-based and trauma-informed services. They use motivational interviewing techniques to establish therapeutic rapport and facilitate active engagement in services. They conduct assessment interviews using the ASAM and DSM-5, They work collaboratively and provide evidence-based therapeutic interventions to address the health service-related needs (HRSNs) and barriers of the client.

They facilitate evidence-based groups and conduct individual counseling sessions, monitoring progress towards identified goals of the client.

Essential Duties And Responsibilities
  • Provides client-centered care utilizing evidence-based and trauma-informed services.
  • Uses motivational interviewing techniques to establish therapeutic rapport and facilitate active service engagement.
  • Conducts assessment interviews using the ASAM and DSM-5.
  • Provides evidence-based therapeutic interventions to address identified goals of the client.
  • Facilitates evidenced based groups.
  • Conducts individual counseling and care coordination sessions.
  • Types and maintains timely and accurate documentation, including group notes, individual notes, status reports and documenting other contacts made concerning the client.
  • Collects and monitors random urinalysis testing.
  • Supports clients in resolving their health service-related needs (HRSN).
  • Works collaboratively with community partners to support clients in accomplishing their goals.
  • Completes monthly status reports for all clients on caseload and facilitates sending them to providers involved with client care.
  • Conducts intake and registration of clients.
  • Provides referrals as needed to support the client.
  • Monitors and tracks the progress of clients in program.
  • Assists clients with applying for Medicaid coverage.
Other Duties And Responsibilities
  • Coordinates and maintains a working relationship with all referral sources.
  • Attends staff meetings and works as a member of a multidisciplinary team to identify and implement quality services.
  • Performs other duties as assigned.
Minimum Qualifications And Experience

Experience, Education and Credentials

  • Associate degree or higher in Human Services or a related field from an accredited college or university.
  • In addition to formal education, the qualified candidate will generally have at least one to three years of drug and alcohol addiction counseling experience.
  • CADC I certification or higher with MHACBO.
Knowledge,

Skills And Abilities
  • Excellent interpersonal communication skills.
  • Strong computer and time management skills.
  • Advanced understanding of ASAM PPC-2R and DSM-5.
  • An intermediate understanding of Oregon Administrative Rules is preferred.
  • Qualified candidate must be able to pass a DHS criminal background check.

Compensation
  • Wage will depend on experience and credentials.
  • An additional stipend is available for Spanish/English bilingual employees following the completion and passing of the Alta Language Test.
