We are seeking a Nurse Practitioner or Physician Assistant to join our interventional pain management team across multiple locations.

The provider will:
- perform comprehensive assessments, diagnoses, and develop care plans
- order and interpret diagnostic studies
- document patient care
- collaborate with team members
- monitor and refer patients as needed
Qualifications include current licensure, national certification, DEA license or eligibility, and BLS. The position is full-time with a salary range of $125,000-$150,000 annually. Relocation to Sutherlin, OR, is required before starting.
