- Maintain accurate record keeping for admitting, transferring, and discharging residents.
- Coordinate daily with nursing staff and support personnel to plan shift services, programs, and activities, ensuring resident care plans are fully executed.
- Prepare and administer medications; review medication cards for completeness and accuracy.
- Review resident care plans for appropriate goals and revisions based on nursing needs.
- Plan, organize, develop, implement, and interpret programs and policies that provide quality care.
- Provide leadership to nursing personnel; conduct daily unit rounds to confirm staff compliance with resident care plans.
- Develop and maintain rapport with nursing staff, other department personnel, residents, family members, and all individuals involved in resident care.
- Participate in facility surveys by authorized government agencies.
- Licensed Practical Nurse (LPN) degree from an accredited college or university.
- Current, unencumbered, active license to practice as an LPN in Oregon.
- Knowledge of nursing and medical practices, laws, regulations, and guidelines applicable to nursing care facilities.
- Chart nurses’ notes in an informative, descriptive manner that reflects care provided and resident response.
- Experience with electronic medical records and computer documentation systems.
- Maintain confidentiality of all resident care information in accordance with HIPAA guidelines.
- Fluent in reading, writing, and speaking English.
- Active CPR/BLS Certification required.
