Evergreen Hospice serves patients throughout Linn, Benton, Marion, Polk, Lincoln, and Tillamook Counties
, delivering care in a variety of settings, including:
- Private residences
- Skilled nursing facilities
- Assisted living communities
- Adult foster homes
Services include intermittent skilled nursing visits, 24‑hour on‑call nursing support, pain and symptom management, medical social services, counseling, spiritual care, hospice aide services, trained volunteers, bereavement support, medications, and medical equipment as appropriate.
Position SummaryThe Hospice Registered Nurse (RN) provides skilled nursing care, case management, and patient and family support to individuals receiving hospice services. The RN collaborates with an interdisciplinary team to coordinate care plans that promote comfort, dignity, and quality of life.
This role requires strong clinical judgment, exceptional communication skills, and the ability to provide compassionate end‑of‑life care across diverse care settings.
Essential Responsibilities- Conduct comprehensive patient assessments and develop individualized plans of care.
- Coordinate and manage hospice services in collaboration with physicians and the interdisciplinary care team.
- Provide skilled nursing care focused on pain and symptom management.
- Monitor patient status and adjust care plans as needed.
- Educate patients, families, and caregivers regarding disease progression, medications, symptom management, and available resources.
- Document assessments, interventions, and outcomes accurately and in a timely manner.
- Participate in care conferences and collaborate with social workers, chaplains, hospice aides, and other team members.
- Provide emotional support and guidance to patients and families throughout the hospice journey.
- Coordinate transitions of care across home, skilled nursing, assisted living, and adult foster home settings.
- Participate in rotating on‑call responsibilities as assigned.
- Maintain compliance with hospice regulations, organizational policies, and quality standards.
- Bachelor of Science in Nursing (BSN).
- Previous home health, hospice, or case management experience.
- Broad‑based nursing experience with demonstrated clinical expertise.
- Experience providing end‑of‑life care in home or community settings.
- Current, unencumbered Oregon Registered Nurse (RN) license.
- Current Healthcare Provider Basic Life Support (BLS) certification.
- Valid driver's license and current automobile liability/property damage insurance as required by state law.
- Reliable personal vehicle maintained in safe operating condition.
