Position OverviewThe Registered Nurse – Home Health, under the direction of the Home Health Clinical Supervision and on order of the physician, is accountable for direct and indirect quality care for patients of all ages, according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act. Professional development is an integral part of this nursing practice.
The position coordinates patient care activities with other members of the health care team in a collegial relationship. Working in Home Health provides the unique opportunity to care for your patients where they are most comfortable.
- Accountable for direct and indirect quality care for patients following the nursing process and the Nurse Practice Act.
- Coordinate patient care activities with other members of the health care team.
- Provide care in home settings ensuring patients’ comfort.
- Oregon Registered Nurse License.
- 1 year medical/surgical experience in an acute care setting.
- 2 years experience in acute care or community mental health setting for Mental Health RN work assignment.
- Recent Home Health experience.
- Hospice or Palliative Care experience for Palliative Care RN work assignment.
- Point of Care experience.
