Role Description The Health and Wellness Director/RN is a full-time, on-site role based in Salem, OR, responsible for overseeing the overall clinical care and wellness programs for residents. Day-to-day responsibilities include assessing resident health needs, developing and updating individualized care plans, and supervising the nursing and caregiving teams to ensure high standards of care. This role coordinates medication management, monitors regulatory and safety compliance, and maintains accurate clinical documentation.

The Health and Wellness Director/RN collaborates with physicians, families, and internal departments, leads staff training on best practices, and supports quality improvement initiatives to enhance resident outcomes and satisfaction.

Qualifications

  • Active Registered Nurse (RN) license in the state of Oregon and prior experience in senior living, long-term care, or a related healthcare setting.
  • Strong clinical assessment, care planning, and care coordination skills, with the ability to manage complex health and wellness needs.
  • Leadership and team management abilities, including staff supervision, coaching, scheduling, and performance feedback.
  • Knowledge of state and federal regulations for senior living, infection control practices, and safety standards.
  • Excellent communication and interpersonal skills for working with residents, families, physicians, and interdisciplinary teams.
  • Proficiency in electronic health records and basic computer applications, with strong attention to detail and documentation accuracy.
  • Ability to prioritize and manage multiple tasks in a fast-paced environment, including participation in on-call rotation as needed.
  • Preferred qualifications include experience in memory care, wellness program development, quality improvement, and relevant certifications such as CPR/First Aid.
