Schedule:
Monday-Friday, 8:00 AM-5:00 PM with a balanced on-call rotation shared across the clinical team.

We're looking for a compassionate Registered Nurse Case Manager to join our growing hospice team. Whether you're an experienced hospice nurse or a home health nurse looking to transition into hospice, you'll be supported by an experienced interdisciplinary team that values collaboration, compassion, and exceptional patient care.

As a Hospice RN Case Manager, you'll provide compassionate, patient-centered care while coordinating services for patients and families throughout their hospice journey. You'll have the opportunity to build meaningful relationships with your patients while working alongside an experienced interdisciplinary team dedicated to providing exceptional end-of-life care.
Responsibilities- Manage an assigned hospice patient caseload from admission through discharge.
- Complete comprehensive nursing assessments and ongoing patient evaluations.
- Develop, implement, and update individualized plans of care.
- Coordinate care with physicians and the interdisciplinary team.
- Provide skilled nursing visits and symptom management in patients' homes.
- Educate patients and families regarding medications, disease progression, and available hospice resources.
- Maintain timely and accurate clinical documentation.
- Supervise Home Health Aides in accordance with state and federal regulations.
- Participate in weekly interdisciplinary team meetings.
- Provide compassionate end-of-life support, including clinical pronouncement of death when appropriate.
- Current Oregon Registered Nurse (RN) license.
- Current CPR certification.
- Valid Oregon driver's license.
- Reliable transportation and current automobile insurance.
- Strong clinical assessment, communication, and critical thinking skills.
- Ability to work independently while collaborating within an interdisciplinary team.
- Hospice or Home Health nursing experience, but not required.
- Experience using electronic medical records (EMR).
