Role And Responsibilities
The LPN/LVN - Home Health provides quality nursing care to patients in their homes and nursing centers under the supervision of a Nurse Case Manager or Clinical Supervisor. They perform duties aligned with the nursing process, ensuring compliance with all state and federal regulations, including the Nurse Practice Act. The role involves 25% to 35% travel to patient locations within designated areas, delivering patient-centered, whole-person care that emphasizes understanding, commitment, and mutual respect.
Qualifications And Skills- Oregon Licensed Practical Nurse certification/license.
- Within 30 days of hire: BLS certification from American Heart Association.
- Minimum of 1 year experience in acute care, home health, public health nursing, or a skilled nursing facility.
- Valid driver’s license and auto insurance, with the ability to travel as required.
- Provide direct patient care in home and facility settings.
- Follow established nursing protocols and procedures.
- Ensure compliance with regulatory standards.
- Travel to patient homes within coverage areas.
