Description
The Clinic Nurse plays an essential role in the success of the Practice by delivering compassionate, high-quality care to patients in the oncology clinic setting. This position has a meaningful impact on patients and their families through skilled education, clinical support, and ongoing guidance throughout treatment.
Working closely with the care team, the Clinic Nurse helps ensure a coordinated and positive experience for patients while supporting strong clinical outcomes. Responsibilities include patient triage, symptom management, education, and care coordination, as well as contributing to quality initiatives that enhance overall care delivery.
Essential Job Functions Patient Care & Triage- Perform clinical triage via phone, portal, and in-person visits using evidence-based protocols.
- Assess, document, and communicate patient-reported symptoms; escalated urgent issues or symptoms to providers and arrange/enter orders as needed.
- Initiate supportive care interventions under established protocols or provider direction.
- Reinforce education on treatment plans, including chemotherapy, immunotherapy, and oral oncology agents.
- Educate patients and families about side effect management, safety precautions, and escalation guidelines.
- Support patients in understanding care plans and navigating the oncology clinic.
- Collaborate with providers, pharmacists, infusion nurses, and external care teams to ensure continuity of care.
- Monitor provider inbox for lab and imaging results; escalation abnormalities to providers per clinic protocol.
- Facilitate referrals in tandem with the medical assistant for prior authorizations and coordination with specialty services as needed
- Provides advocacy for oncology patients within the community and provides networking and referral support as indicated.
- Provides liaison support to patients and primary physicians.
- Documents to support high quality patient care, timely, while communicating clearly, accurately and concisely in the electronic medical record (EMR).
- Participate in quality assurance activities, including QOPI and safety reporting.
- Ensure compliance with clinic protocols, safety standards, and regulatory requirements.
- Serve as a clinical resource for Medical Assistants and administrative staff.
- Coordinates with MA team members to support workloads when needed.
- Works closely with clinic staff including specialty pharmacies, charge nurses, schedulers, financial advocates and others as patient care dictates.
- Participate in team huddles and staff meetings to optimize workflow.
- Support clinic initiatives for process improvement and patient experience enhancement.
- Participates in at least 10 hours per year of educational opportunities related to oncology or hematology nursing.
- Maintains expertise in oncology disease entities, including pathophysiology, and diagnostic, pharmacologic and treatment modalities.
- Provides documentation of professional development.
- Regular, predictable, and reliable attendance isrequiredto perform the essential duties of this position and to meet operational deadlines,maintainteam coverage, and complete time-sensitive work that depends on in-person coordination and access to on-site systems/resources. Employees must report to work as scheduled, be punctual, and remain at work for assigned hours. For roles requiring coordination with patients, providers, and/or teams, consistent availability during scheduled hours is necessary tomaintainsafe and effective operations, continuity of work, andtimelycompletion of job responsibilities.
