Wage: 25.13/hr + DOE + DOQ
Schedule: Monday–Friday, 8:00am–4:30pm

Position SummaryThis position works with participants and is responsible for assisting people to move from unsheltered living situations to emergency shelters and stable housing. The Young Adult Case Manager will regularly visit known camps to build relationships with unsheltered individuals and provide on-site support to participants in the YHEP program. The role collaborates with community partners and participates in joint outreach to help people complete housing assessments, sign up for Oregon Health Plan (OHP), obtain documentation needed to obtain housing, and connect unsheltered individuals to local resources.
The primary focus is case management for young adults (ages 18–24) and may support outreach in one or more areas including case management and outreach for singles, couples, and families, and/or community health work and health and housing integration.
Job Duties Include (Not Limited To)
Data, Documentation, and Compliance
- Maintain thorough and accurate documentation of client interactions, services provided, and progress toward goals.
- Submit all data, case notes, and reporting as required by program and grant guidelines.
- Ensure that all Personal Health Information (PHI) is handled in compliance with HIPAA standards.
- Participate in training initiatives to enhance team capacity.
Relationships and Housing Navigation
- Build relationships and trust with unsheltered individuals and YHEP program participants.
- Support participants to access resources, including Oregon Health Plan, domestic violence intervention, drug and alcohol treatment, behavioral and mental health services, and other supports.
- Conduct regular case manager visits to assess and address needs that may impact housing retention.
- Assist participants in achieving their goals, communicating progress with program manager and peer specialist, and helping remove barriers while maintaining the dignity and choice of the participant.
- Educate individuals on life skills, such as appointment management, nutrition, hygiene, and navigating systems independently.
- Maintain relationships with YHEP participants to ensure smooth transition during after-care case management.
Access to Shelter and Housing Opportunities
- Identify and address barriers to shelter and housing opportunities with participants.
- Provide housing navigation support for unsheltered individuals.
- Support transitions with services or programs to ensure collaboration and continuity of care.
Community Engagement
- Cultivate the mission and values of C@P by developing and working with community partners and Church at the Park programs.
- Network and attend meetings with other social service agencies to promote interagency cooperation and coordination.
- Participate in collaborative outreach with partners such as Salem Housing Authority, HIV Alliance, Easter Seals, ARCHES, Northwest Human Services, and others.
- Collaborate with all team members in a respectful and collaborative manner, following C@P’s values and principles, including compassion, relationship-building, empathetic listening, peacemaking, outward mindset, and non-violent communication.
- Attend all required training, team meetings, and learn policies and procedures for C@P operations.
Work Environment
- Close quarters with vulnerable people, including individuals without stable housing who may have mental health or substance use concerns.
- Indoor/outdoor work with frequent interruptions and occasional unpredictable behavior.
- Driving between Church at the Park sites, partner agencies, and encampments throughout the area.
Other Requirements
- Must have a valid Oregon Driver’s license; must have been a licensed driver for at least 3 years; driving record must permit coverage under the organization’s insurance.
- All applicants must pass a pre-employment drug screening prior to beginning employment.
