Our Youth & Family Services program in Salem is hiring a Care Coordinator to join the team.

The prospective candidate will be working in our Wraparound Program to help orient youth, families, and community partners to the Wraparound team approach, and help families develop individual care plans with the services and supports to meet their needs. Wraparound is guided by 10 principles, family voice & choice, strengths based, team based to name a few, and we are looking for someone who shares these principles and values.

The candidate will also provide safety planning, case management to assist youth/families with access to resources and supports, and collaborate with community partners for coordination of care. GENERAL STATEMENT OF DUTIES

Coordinate care and case management, facilitate team meetings, and develop wraparound care plans incorporating professional and natural supports to meet the intensive, community-based treatment needs of individuals and their families. Provide direct services by conducting strengths and needs assessments, making referrals to appropriate services, supporting system of care development, and assisting with service coordination across partner agencies.

SUPERVISION RECEIVED

Works under the general supervision of a Clinical Supervisor, Health Program Manager, or designee who provides continual or individual assignments, describes tasks, deadlines, and priorities, and gives specific instructions for new, complex, or unusual assignments. The employee uses initiative in conducting recurring assignments. The supervisor ensures the work is technically accurate and complies with instructions or established procedures.

SUPERVISION EXERCISED
  • Orients individuals, families, and teams to program values and process; obtains permission for service and release of information forms; identifies professional and natural supports to participate on the wraparound team and orient these individuals to the wraparound process and explain roles.
  • Develops strength/needs assessment with input from individual, family and involved professional and natural supports for the wraparound meetings and individualized plan of care.
  • Organizes and facilitates wraparound team meetings; develops a wraparound plan of care addressing strengths and needs for individuals and families across multiple domains; facilitates team meetings with a consensus-based collaborative decision-making process; plan of care will include professional and natural supports to meet the intensive community-based treatment and support services needs of youth and their families; ensures that the team meetings and all supporting mental health services are culturally and linguistically responsive, person-centered, and emphasizes individual and family voice and choice in the decision-making process;

    completes a functional behavioral assessment and develops a crisis/safety plan; facilitates the collaborative development, periodic review and revision of the wraparound plan of care, including a proactive crisis and safety plan; ensures shared accountability for all team members and is responsible for the development of outcome-based goals and measurement strategies; provides high quality wraparound services in accordance to the model;

    adheres to system of care values, principles of wraparound, and supports a strength-based approach at all levels.
  • Provides service coordination and case management to assist families in accessing formal and informal supports and services; facilitates communication between the family, natural supports, community resources, and involved youth-serving providers and agencies; facilitates and participates in team meetings at which strengths and needs are identified and safety planning occurs; participates in intra-agency and interagency case staffing meetings for purposes of evaluation and treatment planning for individuals;

    provides for continuity of care by creating linkages to, and managing transitions between levels of care for youth, and transitions of older youth to the adult service system; serves as an advocate/liaison in assisting individuals to obtain necessary services; supports the development of a system of care and assists in service coordination with partner agencies; participates in all fidelity reviews, coaching and meeting observations.
  • Maintains practice and documentation in accordance with Health department, high-fidelity programs and OARs; documents services to meet grant and agency requirements.
