Summary

The IEP and Transition Coordinator will provide special education professional expertise to the teaching faculty and administration of Hope Tree Academy. He/she will also assist the Director of Education and Education Supervisor in overseeing student scheduling, testing, and a variety of transition (job placement, career exploration, employability skills development) activities.

Experience & Education

The position requires no less than a master’s degree in special education from an accredited college or university. The position also requires at least five years of experience working with student populations in alternative, special education or non‑traditional settings and an endorsement in special education general curriculum K‑12. The IEP and Transition Coordinator must be able to demonstrate professional leadership and knowledge of special education concepts;

implementation strategies and processes applicable to a middle/secondary school environment educating residential and day disabled "at risk" youth.

Essential duties include the following. Other duties may be assigned as needed.

  • Consults with and assists the Education Assessment Coordinator in the procurement of and administration of SOL testing given to HTA students.
  • Consults/assists in student scheduling by coordinating courses with students Individualized Education Plan (IEP or 504 Plan).
  • Oversees IEP's and IIP's and coordinates IEP/IIP meetings.
  • Serves as the IEP liaison between HTA and local education agencies in providing written information for IEP team.
  • Ensures IEP due dates, eligibility dates, goals and objectives, progress reports, etc., are being completed and met in a timely manner by Special Education Teachers.
  • Ensures all student documentation is filed in cumulative folder.
  • Oversees submission of HTA progress reports to local education agencies.
  • Reviews the physical copy of the IEP, ISDP and ensures documentation is in student’s cumulative file.
  • Connects students to agencies (Workforce Development, Department of Rehabilitation Services, Blue Ridge Assisted Living, etc.) to assist with transition plans and post‑secondary goals.
  • Assists with accommodations with on‑the‑job training experiences.
  • Consults/assists the administration with a variety of transition activities for HTA's secondary students…i.e. job placements, apprenticeship placements, career exploration field trips and employability skills development.
  • Visits On‑the‑Job training sites.
  • Leads and oversees Element 3 meetings and ensures all components are submitted on time.
  • Establishes appropriate priorities and uses time efficiently in carrying out his/her job responsibilities.
  • Assist with the admission process to ensure IEPs/504 Plans meet VDOE and VAISEF regulatory requirements for student admission.
  • Provide written IEP information for Local Education Agency and is responsible for developing Individualized Instruction Programs.
  • Attends a minimum of 36 hours per year of professional development training experiences to enhance professional abilities as required by licensure recertification standards.
  • Establishes appropriate priorities and uses time efficiently in carrying out his/her job responsibilities.
  • Commitment to implement Trust Based Relational Intervention (TBRI) strategies throughout the school day with students to ensure students have been given every opportunity to make good decisions.
