PRN Home Health Registered Nurse

PRN Home Health Registered Nurse position at Wesley Healthcare at Home (HCA Healthcare). Provide coordinated skilled nursing care to patients of all age groups in the home. Work under the supervision of the Clinical Manager and collaborate with interdisciplinary team to achieve patient goals.

• Assess home care patients, identifying physical, psychosocial and environmental needs based on documentation, clinical records, case conferences, team reports, call‑in logs and on‑site evaluations.
• Complete OASIS, assessment and visit paperwork per agency policy and ensure clinical notes accurately reflect communication and coordination with physicians and interdisciplinary team.
• Communicate significant findings, problems and changes to Clinical Manager and physicians, documenting all findings and interventions.
• Supervise and provide clinical direction to home health aides and LPNs/LVNs to ensure quality and continuity of services.
• Participate in on‑call rotation and emergency call per agency policy.

• Registered Nurse (RN)
• Basic Cardiac Life Support (BCLS) obtained within 30 days of employment start date
• Driver’s license (DL)
  Associate Degree or Registered Nurse Diploma
• One year of experience required (or specify required experience if applicable)
