Job Overview – Clinical Nurse Coordinator Compensation: $85,000/year + bonus

TAG Med Staffing is hiring a Clinical Nurse Coordinator in Ford County, KS for our client, supporting wound care coordination, patient management, and referral development initiatives within a clinical healthcare environment. This dual-function role combines direct patient care responsibilities with community outreach and referral relationship management to support patient access to wound care services. The ideal candidate is clinically strong, autonomous, and experienced within wound care, case management, or care coordination environments.
Responsibilitiesas the Clinical Nurse Coordinator:
- Patient Care Coordination: Provide direct patient care, wound assessments, treatment coordination, and ongoing monitoring of patient progress.
- Wound Care Management: Develop and oversee individualized wound care plans while coordinating follow-up care, diagnostics, and treatment documentation.
- Referral & Marketing Outreach: Build and maintain relationships with clinics, healthcare providers, and referral sources to support patient referrals and community engagement.
- Clinical Operations: Support quality management initiatives, clinical documentation reviews, and compliance with healthcare standards and procedures.
- Cross-Functional Collaboration: Partner with physicians, healthcare teams, facility leadership, and referral sources to ensure continuity of patient care.
- Education & Support: Provide clinical education, patient support, and ongoing communication with patients, providers, and internal care teams.
- Education: Associate’s or Bachelor’s degree in Nursing is required.
- Certifications: Active Registered Nurse (RN) license and current BLS and ACLS certifications are required.
- Experience: Minimum of 3 years of nursing experience, wound care experience, case management experience, or a combination of related clinical experience is required.
- Industry Knowledge: Experience with in wound care, care coordination, outpatient clinical environments, or healthcare marketing is preferred.
- Skills & Attributes: Strong communication skills, relationship-building abilities, organizational skills, clinical judgment, autonomy, and the ability to manage field-based responsibilities independently are required.
