RN - Case Management

Monday to Friday, 8‑hour shifts (8A – 5P)

We are seeking a compassionate and dedicated Case Management Registered Nurse to join our team. The ideal candidate will have expertise in discharge planning and observation management, with a focus on providing exceptional patient care. You will collaborate closely with multidisciplinary teams to ensure smooth transitions and continuity of care.

• Conduct assessments for discharge planning and patient needs.
• Manage observation cases effectively following established protocols.
• Coordinate care with healthcare teams and community resources.
• Document patient progress and care plans in Midas, Meditech, and Well Sky.
• Ensure compliance with interqual criteria and hospital policies.

• Current RN license in the state of practice.
• Experience in case management and discharge planning is essential.
• Advanced certifications in case management preferred.
