Candidate Pay Rate: $67 per hour
Roles & Responsibilities- Provide patient-centered evaluation and skilled physical therapy interventions to support recovery and functional independence
- Develop and update individualized treatment plans based on assessment findings and patient goals
- Educate patients and families on home programs, safety strategies, and progress expectations
- Collaborate with nurses, physicians, and rehabilitation staff to coordinate care and optimize outcomes
- Document assessments, treatment plans, progress notes, and discharge summaries accurately in the medical record
- Monitor and adjust interventions based on patient response and evidence-based practice
- Participate in team huddles and care planning to ensure continuity and quality of care
- Current, unrestricted California Physical Therapist license (or appropriate PTA license if applying as PTA)
- Active BLS/CPR certification
- Demonstrated ability to manage a varied caseload in an inpatient rehabilitation or skilled setting
- Strong interpersonal and communication skills to work with patients, families, and the care team
- Willingness to work a schedule that may include weekends and rapid start dates
- Doctor of Physical Therapy (DPT) preferred for PT roles
- Associate degree or accredited PTA program completion for PTA roles
- Additional certifications in specialty rehab skills are a plus
- Skilled in adult rehabilitation assessments and therapeutic exercise prescription
- Experience with transfers, gait training, balance and mobility training
- Proficient documentation and goal-setting aligned with payer and facility requirements
- Ability to use clinical reasoning to progress patients toward functional goals
- Comfortable collaborating in a multidisciplinary rehab environment
- Familiarity with common assistive devices and adaptive equipment
