The Speech-Language Pathologist will provide evaluation and treatment services to SNF patients, working collaboratively with an interdisciplinary care team to support patient-centered rehabilitation outcomes.

• Evaluate and treat patients in a Skilled Nursing Facility setting
• Develop and implement individualized treatment plans
• Collaborate with nursing, therapy, and medical teams
• Complete timely and accurate documentation per facility standards
• Utilize Net Health for documentation

• Active California SLP License (required)
• SNF experience preferred (mentorship available if experience is limited)
• Strong documentation and time-management skills
• Flexible, collaborative, and patient-focused approach
• Ability to float to a sister facility within walking distance if needed
