Physical Therapy Position
Conduct Physical Therapy assessments to determine the Physical Therapy care needs, preferences and goals of the participants and actively participate in interdisciplinary team meetings to develop participant care plans. Deliver and document Physical Therapy interventions in accordance with physician orders and/or as agreed upon in the participants' care plans including but not limited to providing active and passive exercise, muscle re-education, gait and functional training of activities of daily living;
treatments such as massage, heat, sound, water light or electricity. Supervise the Therapy Assistants including providing training, instruction and guidance needed to implement Functional Maintenance Programs. Provide training and instructions to staff, participants and caregivers/families in the use and care of therapeutic equipment. Order, track and manage maintenance for equipment. Remain alert to pertinent input from other team members, participants, and caregivers and update the interdisciplinary team promptly of any changes in participants' condition or functional status.
Work with the other members of the interdisciplinary team to support smooth care transitions between settings (e.g., hospitals, skilled nursing facilities, home, etc.). In coordination with the Marketing Team, support enrollment of prospective participants into the program. Participate in end-of-life care coordination and support.
Education/Training/
Certifications:
Bachelor's Degree in Physical Therapy plus five years' of progressively responsible PT experience. Master's Degree in Physical Therapy or Doctorate in Physical Therapy preferred. Valid California Physical Therapy License, or proof of License eligibility. The qualifying Physical Therapy Degree earned outside of the US must be evaluated to the US equivalent to a Degree in Physical Therapy. Valid State Driver's License with a clean DMV record.
Copy of recent Vehicle Insurance required Compensation: $45.00 - $60.00 per hour Minority and Women Owned Business Enterprise (MWBE) certified by the State of New Jersey and Minority Business Enterprise (MBE) certified from National Minority Supplier Development Council.
