Department: Outpatient Pharmacy

The Pharmacy Medication Reconciliation Technician reports directly to the Director of Pharmacy or his/her designee and receives support from the Pharmacy Transitions of Care Coordinator. The Technician assists pharmacists in obtaining the most accurate prior‑admission medication lists by interviewing patients and consulting with their outpatient pharmacies. The Technician performs medication histories in the Emergency Department and inpatient areas, helping to prevent medication errors caused by inaccurate or incomplete information.

The Technician is expected to provide exemplary service to internal customers and embody SVMHS’s STAR values in all interactions with patients and colleagues.

• Collaborates with unit care providers to determine when patients can be interviewed and obtains medication histories on all assigned patients.
• Establishes rapport with patients to obtain medication history or determines if a patient or family member can provide medication history.
• Has the ability to navigate an electronic Health database and enter new medication histories or update existing medication profiles.
• Identifies medication names, common dosages, dosage forms and frequency of administration for all medications.
• Assesses incompleteness of current medication lists by examining drug, dose, frequency, and indication for use.
• Determines time last dose of each medication was taken prior to admission for future scheduling.
• Determines compliance of medication consumption prior to admission.
• Obtains indication for PRN medications and actual frequency of use of PRN medication.
• Prompts patients for additional medications, specifically suggesting over‑the‑counter (OTC) medications including herbals, vitamins, pain relievers, cold products, patches, lotions & creams, eye drops, sprays, and inhalers.
• Calls outpatient pharmacies or other healthcare facilities to confirm medication regimens or investigate discrepancies when necessary.
• Determines if patients were on recent antibiotics prior to admission.
• Verifies accuracy of medication history by source confirmation.
• Obtains medication allergy history and related reactions to the medication.
• Maintains confidentiality of all designated hospital and patient information and complies with HIPAA standards at all times.
• Conducts interpersonal relationships with patients, visitors, physicians and fellow employees in a courteous and customer‑friendly manner.
• Follows established hospital infection control and safety procedures.
• Performs other duties as assigned.

Education: A minimum of a High School Diploma is required. Successful completion of an accredited Pharmacy Technician certification course is required.

Licensure: Current California Pharmacy Technician certification is required.

Experience: A minimum of four (4) years’ experience in retail pharmacy is required. Excellent knowledge of brand and generic medications is required. Ability to communicate with health care team members and patients of all education levels is necessary.

The hourly rate for this position is $34.36 – $37.89. The range displayed on this job posting reflects the target for new hire salaries for this position.
