Full Time Physical Therapist Case Manager

Location: Salinas, CA

Job Overview

In partnership with other clinicians on the case, you will provide skilled services to evaluate and address the needs of individuals in their homes, so they can live safely and productively in their environment.

Responsibilities
  • Perform an evaluation in the home and develop a care plan, working with PTAs to follow the plan.
  • Help patients with mobility and movement to regain their independence.
  • Create detailed treatment and care plans for each patient.
  • Communicate and coordinate with patients’ physicians to ensure a high level of care.
  • Evaluate and report patients’ progress to MDs.
  • Assist with documentation and administrative tasks.

Requirements
  • Current unrestricted California Physical Therapist License.
  • Current BLS/CPR Certification.
  • Valid California Driver's License.
  • Knowledge of local community resources.
  • Knowledge of individual development and human behavior as it relates to illness/injury and cultural influences on healthcare.
  • Knowledge of state and federal homecare regulatory guidelines (preferred).
  • Experience in home health care (highly preferred).
  • Experience working with high acuity patients (plus).
  • Experience with an interdisciplinary team (highly desired).
  • Case Management fundamentals (highly desired).
  • Self‑directed, empathetic, critical thinker, outstanding interpersonal skills, leadership skills, organized.
