Responsibilities
- Assist in optimizing patient drug regimens, maintaining appropriate records for medications and pharmaceutical supplies.
- Perform drug regimen review including identifying therapeutic duplications; reviewing drugs, allergies, drug‑drug, drug‑nutrient, and drug‑laboratory test interactions; evaluating doses for age, renal and liver function; assisting with patient/family education.
- Monitor patients on clinical drug protocols; attend codes; attend rounds and patient‑care conferences as needed.
- Assist with pain management as needed; provide drug in‑services to nursing, pharmacy, medical staff, and ancillary personnel.
- Identify the presence or potential of adverse drug events and report as necessary.
- Perform other duties as assigned.
BA/BS in Pharmacy or MS/PharmD preferred.
Current California Pharmacy License required.
Current BLS/Healthcare Provider status per American Heart Association required.
Experience in hospital or healthcare setting preferred.
Must have motor skills to prepare, mix, and transfer drugs aseptically, especially chemotherapy, and perform data entry.
Benefits and CompensationHourly rate: $89.99 - $109.39.
Union: NUHW.
Work shift:
Variable.
FTE: 0.9.
Scheduled hours: 35.
