Physical Therapist
Physical therapist provides individualized care to help patients regain mobility, independence, and quality of life in a collaborative rehabilitation setting. This full-time role supports a multidisciplinary team in delivering evidence-based interventions and fostering patient-centered care.
Key Responsibilities:- Evaluate patients and develop individualized treatment plans based on physician orders and goals.
- Perform comprehensive assessments and administer evidence-based interventions.
- Promote functional mobility, strength, balance, gait, endurance, and independence through tailored care plans.
- Provide safe, compassionate care across all age groups.
- Complete accurate, timely documentation for evaluations, sessions, progress notes, and discharge plans.
- Assign and supervise PTAs and rehab aides to support efficient patient care.
- Review and update treatment plans for patients served by support staff.
- Educate patients, families, caregivers, and staff on treatment plans, home exercises, and injury prevention.
- Assist with orientation of new employees, volunteers, and supervise PT students and PTA students during clinical affiliations.
- Assume Charge Therapist responsibilities as needed.
- Coordinate care with physicians, nurses, case managers, and other rehabilitation professionals.
- Adhere to safety policies, regulatory standards, and professional practice guidelines.
- Graduate of an accredited Physical Therapy program.
- Current California Physical Therapist License or eligibility for California licensure.
- Current BLS (Basic Life Support) for Healthcare Providers certification through the American Heart Association.
- Strong knowledge of evidence-based physical therapy practices and patient-centered care.
- Excellent communication, organizational, and interpersonal skills.
- Ability to work independently while collaborating within a multidisciplinary healthcare team.
- Previous Physical Therapist experience not required; new graduates encouraged to apply.
