Responsibilities
  • Collect all discharged patient medical records daily
  • Prepare medical records and loose filing for scanning, match batch header sheets, perform quality review, upload and accept batches for completeness
  • Scan and index health and administrative information into the electronic health records system within established time frames
  • File or destroy paper records after scanning, per policy and procedure
  • Prepare ED, hospital, and consultant records for scanning: remove staples, place batch header sheets with appropriate medical record, and organize records orderly
  • Interact closely with the Information Technology Department when discrepancies occur involving the computer system
  • Maintain an error rate of 5% or less in scanning medical information into the electronic health records system
  • Combine current patient medical record with any previous discharges, if applicable
  • Assist in quality audits to ensure the integrity of scanned images into the electronic health records system
  • Place assembled medical records in the proper folder by record number and scan them into the Electronic Health Record (EHR)
  • Analyze medical records of discharged patients for completeness and accuracy according to department and health facility policy
  • Monitor deficiencies in the electronic health record that require physician completion
  • Forward all analyzed medical records to coding staff
  • Coordinate release of medical information to all requesters, including patients, physicians, and other healthcare providers
  • Verify authorizations in accordance with health facility policy and procedures and state and federal laws
  • Maintain security and confidentiality regulations for record access and review; consult director or supervisor of any unusual request
  • Stamp all requests for records with date received and log them in the correspondence computer
  • Prepare requests for copying service in accordance with guidelines and notify requesters when records are available
  • Maintain the supply closet and order supplies as necessary
  • Perform performance improvement audits on each medical record through data collection and documentation review as directed by the director
  • Answer telephone calls and take accurate messages
  • Be familiar with pediatric and OB/GYN medical record requirements
  • Maintain health facility confidentiality standards and policies
  • Assist in retrieving medical records for physician completion, as applicable
  • Accept additional assignments and complete them within a designated timeframe
  • Keep up with workload and maintain a good working relationship within the department and with other departments
  • Protect patient confidentiality and secure computer files and access codes
  • Support and maintain a culture of safety and quality
Qualifications Professional Requirements
  • Adhere to dress code; appearance must be neat and clean
  • Complete annual education requirements
  • Maintain regulatory requirements, including all federal, state, local regulations, and accrediting organization standards
  • Maintain patient confidentiality at all times
  • Report to work on time and as scheduled; complete work within designated time
  • Wear identification while on duty and use the computerized punch‑time system correctly
  • Complete in‑services and return them in a timely fashion
  • Attend annual reviews and department in‑services as scheduled
  • Attend at least 80% of staff meetings annually and read and return all monthly staff meeting minutes
  • Represent the organization in a positive and professional manner
  • Actively participate in performance improvement and continuous quality improvement (CQI) activities
  • Comply with all organizational policies regarding ethical business practices
  • Communicate the mission, ethics, and goals of the health facility, as well as the department focus statement
Regulatory Requirements
  • High School graduate or equivalent
  • Two or more years of previous similar health facility experience
  • Knowledge of accrediting organization standards and state medical record requirements
Language Skills
  • Able to communicate effectively in English, both verbally and in writing
  • Additional languages are preferred
Skills
  • Basic computer knowledge
