Pacific Coast Post-Acute is seeking a compassionate, skilled, and motivated Physical Therapist (PT) to join our in-house rehabilitation team in Salinas, California. As a leading short-term rehabilitation and skilled nursing facility in Monterey County, we provide advanced sub-acute and restorative care designed to help residents regain mobility, independence, and quality of life. Our rehabilitation program features a spacious therapy gym and a dedicated ADL training suite, including a functional kitchen, laundry area, and tub/shower setup. These resources allow therapists to incorporate practical, real-world activities into treatment and prepare residents for a safe return home.

Develop individualized treatment plans based on each resident's goals, medical needs, and functional abilities. Apply evidence-based interventions while maintaining a strong focus on quality outcomes and resident-centered care.

Collaborative Team Environment

• Work alongside an experienced interdisciplinary team that may include Physical Therapists, Physical Therapist Assistants, Occupational Therapists, Speech‑Language Pathologists, nurses, physicians, and other healthcare professionals.

• Perform comprehensive physical therapy evaluations and develop individualized treatment plans based on resident goals and functional needs.
• Provide skilled interventions, including gait training, therapeutic exercise, balance training, manual therapy, and neuromuscular re‑education.
• Monitor resident progress and modify treatment plans as appropriate to maximize functional outcomes.
• Guide residents through their rehabilitation journey, from acute recovery through safe discharge and community reintegration.
• Collaborate with the interdisciplinary care team to coordinate effective, resident‑centered care.
• Supervise Physical Therapist Assistants (PTAs) in accordance with applicable California practice requirements.
• Maintain accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries.
• Participate in care conferences, clinical meetings, and quality improvement initiatives.
• Communicate effectively with residents, families, caregivers, and other members of the healthcare team.
