Master's degree from an accredited college or university in occupational therapy or a related field One (1) year experience providing occupational therapy to children with various disabilities in an educational, rehabilitative or related setting
- Certificate Copy (Occupational Therapist Certification issued by the National Board for Certification in Occupational Therapy (NBCOT))
- Copy of Transcript (Master's degree from an accredited college or university in occupational therapy or a related field)
- Credential Copy (State Occupational Therapist License, Occupational Therapist Certification)
- Driver's License Copy (California Driver’s License with evidence of insurability)
- Letter of Introduction
