About Us

Salinas Valley Post Acute is a dedicated skilled nursing and post-acute care center focused on delivering compassionate, patient-centered care. Our team works collaboratively to support residents and their families through every stage of recovery. We are looking for a motivated and detail‑oriented LVN to join our Case Management team.

Position Summary

The Case Manager LVN is responsible for coordinating resident care plans, managing transitions of care, ensuring regulatory compliance, and acting as a key liaison between residents, families, healthcare providers, and interdisciplinary team members. This role is ideal for LVNs who enjoy advocacy, organization, and communication as much as clinical work.

Key Responsibilities
  • Coordinate and manage individualized care plans for residents
  • Facilitate communication between physicians, nursing staff, therapists, and families
  • Assist with discharge planning and transitions of care
  • Monitor resident progress and update care plans accordingly
  • Ensure documentation meets state, federal, and facility standards
  • Participate in interdisciplinary team meetings
  • Provide education and support to residents and families
  • Maintain accurate and timely records and reports
Qualifications
  • Current California LVN license in good standing
  • Prior experience in skilled nursing, post‑acute care, or case management preferred
  • Strong communication, organization, and follow‑through
  • Knowledge of care planning, MDS process, and discharge planning a plus
  • Ability to work independently and collaboratively within a team
  • Compassionate, professional, and resident‑focused approach
