JOB DESCRIPTION

The Registered Nurse (RN) for the Assertive Community Treatment Team (ACT) will assist and provide services to adults with serious and persistent mental illness in the community where they live, instead of a hospital or clinical settings. The ideal candidate will assist adults residing in Salinas and South Monterey County with serious mental illness and serious functioning impairments, new to services, not engaged with services, and/or difficulty connecting to system’s services due to psychosocial and/or other barriers.

The incumbent will provide culturally linguistic appropriate services while collaborating with other care providers and health team members. The RN coordinates medication prescribed by the Act Psychiatric provider.

COMPENSATION

$52.65 - $68.56 per hour. Non-Exempt; 40 hours/week;
Eligible for overtime. Compensation based on education and experience.

REPORTS TO

Nurse Administrator.

QUALIFICATIONS Required

Bilingual in Spanish. Current California Registered Nurse license;
Experience in the Mental Health field. Knowledge of psychiatric disorders and psychotropic medications.

Driver’s license for at least two years and driving record acceptable to insurance carrier.

Valid California driver's license; auto in safe operating condition; auto liability insurance; good driving record; criminal record clearance; proof of authorization to work in the United States as required by Immigration and Reform Act of 1986. Ability to work independently with minimal supervision.

Preferred

BSN;
Psychiatric nursing experience;
Medi-Cal documentation experience; proficient in Outlook email correspondence and computer software. Ability to work as a member of a team; accept supervision; maintain appropriate boundaries while working in close inter-personal relationships; learn and apply principles of social rehabilitation; organized, responsible, flexible, empathic, and able to relate effectively with clients, other staff, professionals and members of the community. Knowledge of community resources.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Provide on-call services (when scheduled).
  • Provide culturally linguistic appropriate, recovery oriented, trauma informed, wellness centered services focused on nursing support for patients in homes and other environments.
  • Assist patients with self-administration of medication: knowledge of medication classification and side-effects and expected therapeutic responses.
  • Provide recovery-oriented, wellness centered approach to consumer’s substance use.
  • Coordinate medications prescribed by the Act Psychiatrist Psychiatric provider.
  • Assist Psychiatrist Psychiatric provider as needed to provide patient care.
  • Provide services in differing environments, such as consumers’ homes, schools, gathering places, homeless shelters, etc., including community-based outreach and engagement.
  • Assist clients in attaining community stability and reaching their health, recovery and rehabilitation goals, including helping clients to find and keep employment.
  • Assist clients with their physical and mental health recovery process by developing the skills necessary to lead independent or interdependent, healthy, and meaningful lives in the community.
  • Provide psychiatric and other education and advocacy to patients, patients’ families, and other support members as appropriate.
  • Provide community-based outreach and engagement in homes and other environments.
  • Administer IM medications as needed and document.
  • Conduct and utilize client needs assessments for development and tracking of client centered treatment planning.
