Community Support Facilitator II

Hope Services is seeking a passionate and dedicated Community Support Facilitator II to join our team. This role is pivotal in delivering high-quality support and services to individuals with developmental disabilities across the Bay Area. As part of our commitment to enhancing the quality of life for our clients, the Community Support Facilitator II will work to empower clients, promoting independence and involvement in the community through tailored programs that include job training, therapy, and assisted living support.

Responsibilities

• Develop and implement individual service plans that promote self-advocacy, independence, and community integration for clients with developmental disabilities.
• Collaborate with therapists, healthcare providers, and vocational trainers to coordinate services and maximize client outcomes.
• Facilitate group sessions and workshops to enhance social skills and community engagement among clients.
• Monitor and report on client progress, adapting plans as necessary to meet their evolving needs.
• Ensure compliance with all federal, state, and local regulations, as well as organizational standards in the delivery of service.
• Act as a liaison between clients, families, and community resources to support client goals and needs.
