Advanced Practice Clinician - Nurse Practitioner or Physician Assistant
Location: Wicomico & Accomack Counties, Maryland
Schedule: Monday-Friday, 8:00 am-5:00 pm
Hours: 40 per week
Duration: Potential hire
Dress Code: Medical attire
We are seeking an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to support the Senior Community Care (SCC) program, a longitudinal, integrated care model serving high‑risk members within long‑term care settings. This role is part of an interdisciplinary care team focused on delivering coordinated, patient-centered clinical care in facility environments.
This position offers a strong work-life balance with no on-call, no weekends, and no holidays

- Conduct comprehensive patient assessments
- Manage complex acute and chronic medical and behavioral health conditions
- Ensure accurate and complete documentation, including ICD‑10 coding
- Collaborate closely with the interdisciplinary care team
- Lead advanced illness and advance care planning discussions
- Provide patient and caregiver education and counseling
- Participate in facility-based clinical activities and care planning
- Active and unrestricted state NP or PA license
, or ability to obtain by start date - Master’s degree or higher (NP or PA)
- National board certification:
- NP: AANP or ANCC (FNP, ANP, GNP, or AGACNP)
- PA: NCCPA certification
- Active DEA licensure/prescriptive authority
- Ability to lift a 30‑lb clinical bag, climb stairs, kneel, and perform hands‑on assessments
- Reliable transportation for daily travel
- Ability to obtain a collaborative practice agreement if required in the state
- Full COVID‑19 vaccination (per federal, state, and facility requirements)
- Must pass pre‑employment drug screening
- 1+ year of clinical experience (long‑term care preferred)
- Experience supporting patients with complex behavioral, social, or functional needs
- Strong understanding of geriatrics
, chronic illness, and end‑of‑life discussions - Proficiency in electronic and written medical documentation
- Strong communication, teamwork, and time‑management skills
- Travel Required: Yes, during the work shift
