This Home Health Registered Nurse opportunity is based in Salisbury, Maryland, a welcoming city in the heart of the Eastern Shore. In this role, you will provide skilled nursing care to patients in their homes across Salisbury and surrounding Eastern Shore communities. You will manage a diverse caseload, typically focused on adult and geriatric patients with chronic conditions, post-acute needs, and rehabilitation goals.

Your responsibilities will include comprehensive assessments, individualized care planning, wound care, medication and symptom management, and patient and family education. You will be a key member of an interdisciplinary team, collaborating closely with therapists, aides, and other clinicians to support safe, high-quality care in the home. A typical day involves driving to patient residences within a defined territory, conducting 4 to 6 visits per day depending on visit type and acuity.

You will perform head-to-toe assessments, update care plans, coordinate with providers, and document in the EMR using a laptop or tablet in real time. You may also complete start-of-care assessments, routine follow-up visits, discharges, and occasional on-call or urgent visits based on agency needs. Patient ratios are structured to allow meaningful time with each patient and appropriate travel between homes.

The care environment is focused on continuity and relationship-based nursing, allowing you to truly get to know your patients, their families, and the home environments that impact their health. You will have the support of a clinical leadership team, case conferences, and field-based resources, fostering a collaborative and supportive workplace culture. Orientation and ongoing education are provided to help you stay current on best practices in home health.

Shifts are generally daytime hours on weekdays, with some flexibility based on patient needs and agency scheduling. Rotating weekend and on-call expectations may be included, shared among the team to balance workload. This structure offers a degree of autonomy and independence, along with predictable patterns that support work-life balance.
