, the Journey Coordinator serves as a key resource for residents and families, helping ensure a smooth, positive experience from admission through discharge.

Responsibilities for Journey Coordinator:
  • Serve as a primary point of contact for residents and families on the memory care unit by providing education, answering questions, and addressing concerns throughout their stay.

  • Collaborate with nursing, therapy, social services, admissions, dietary, and other interdisciplinary team members to ensure residents' needs are met.

  • Assist with admission and discharge planning, including coordinating services, documentation, and follow-up care.

  • Facilitate communication between residents, families, physicians, and the interdisciplinary team to promote continuity of care.

  • Conduct resident and family satisfaction rounds and assist in resolving concerns promptly and professionally.

  • Support care conferences and participate in interdisciplinary meetings to advocate for resident-centered care.

  • Maintain accurate documentation and ensure compliance with all facility policies and applicable federal and state regulations.

  • Assist with resident engagement initiatives and programs that enhance the overall resident experience.

  • Perform other duties as assigned to support exceptional resident care and customer service.

Qualifications for Journey Coordinator:
  • Must be a Licensed LPN in the state of Maryland

  • Previous experience in a Skilled Nursing, Rehabilitation, Long-Term Care, or Assisted Living setting preferred.

  • Excellent communication, customer service, and interpersonal skills.

  • Strong organizational skills with the ability to manage multiple priorities in a fast-paced environment.

  • Ability to build positive relationships with residents, families, physicians, and interdisciplinary team members.

  • Proficiency in Microsoft Office and the ability to learn electronic medical record (EMR) systems.

  • Knowledge of resident rights, HIPAA, and healthcare confidentiality standards preferred.
