, the Journey Coordinator serves as a key resource for residents and families, helping ensure a smooth, positive experience from admission through discharge.
Responsibilities for Journey Coordinator:
- Serve as a primary point of contact for residents and families on the memory care unit by providing education, answering questions, and addressing concerns throughout their stay.
- Collaborate with nursing, therapy, social services, admissions, dietary, and other interdisciplinary team members to ensure residents' needs are met.
- Assist with admission and discharge planning, including coordinating services, documentation, and follow-up care.
- Facilitate communication between residents, families, physicians, and the interdisciplinary team to promote continuity of care.
- Conduct resident and family satisfaction rounds and assist in resolving concerns promptly and professionally.
- Support care conferences and participate in interdisciplinary meetings to advocate for resident-centered care.
- Maintain accurate documentation and ensure compliance with all facility policies and applicable federal and state regulations.
- Assist with resident engagement initiatives and programs that enhance the overall resident experience.
- Perform other duties as assigned to support exceptional resident care and customer service.
- Must be a Licensed LPN in the state of Maryland
- Previous experience in a Skilled Nursing, Rehabilitation, Long-Term Care, or Assisted Living setting preferred.
- Excellent communication, customer service, and interpersonal skills.
- Strong organizational skills with the ability to manage multiple priorities in a fast-paced environment.
- Ability to build positive relationships with residents, families, physicians, and interdisciplinary team members.
- Proficiency in Microsoft Office and the ability to learn electronic medical record (EMR) systems.
- Knowledge of resident rights, HIPAA, and healthcare confidentiality standards preferred.
