Contract - PT - Home Health Physical Therapist
• Mon-Fri; 8a-4:30p
• Home Visits Therapy
HCHB charting for visits
Responsibilities- Conduct comprehensive physical examinations
- Assess patient s movement patterns, strength, and range of motion
- Diagnose physical impairments and develop individualized treatment plans
- Design and administer therapeutic exercises, manual therapy techniques, and modalities (e.g., ultrasound, electrical stimulation)
- Educate patients on self-care strategies, home exercises, and injury prevention
- Collaborate with other healthcare professionals, such as physicians, nurses, and occupational therapists
- Monitor patient progress, adjust treatment plans as needed, and discharge patients when appropriate
- Provide patient education on health conditions, treatment options, and lifestyle modifications
- Master s or Doctoral degree in Physical Therapy from an accredited program
- National Physical Therapy Examination (NPTE) license
- State-specific licensure requirements
1 year PT, home health
HCHB experience
