Responsibilities- Work the RAI process and conduct assessments and care plan coordination for assigned residents.
- Complete the MDS, CAAs and care plans within regulated time frames.
- Assess residents through physical assessment, interview and chart review.
- Discuss resident care needs with caregivers, including physician, nursing, social services, therapy, dietary, and activity staff.
- Review information from hospital, consults and outside agencies and use such information in the completion of the assessment and care planning.
- Coordinate, identify, and/or initiate significant change MDS.
- Be prepared to conduct PPS meetings, maintaining MDS assessments per Medicare schedule and maintaining PPS board for monitoring of Medicare days and RUGs utilization in the absence of the Care Management Coordinator.
- Remain current with the American Association of Nursing Assessment Coordinators (AANAC) requirements.
- Registered Nurse, RN
- AANC certification a plus. RAC-CT
- Knowledge of the Resident Assessment Instrument (RAI) process, including the principles of the Prospective Payment Process (PPS) strongly preferred.
- Experience as an MDS Nurse.
