Position: In Home Healthcare LVN- High Acuity (Weekend Shifts)

Position

Join a Company that Puts People First! We are looking for a Licensed Practical/Vocational Nurse (LPN/LVN) to provide skilled nursing care to children and adults in San Antonio, TX. Shift schedule:
Saturday–Sunday, 7 a.m.–7 p.m.

Patient acuity includes feeding tube management, tracheostomy, ventilator support, and medication administration for children (Age Range: Child). Training is available for necessary skills. Experience with pediatric care is preferred.

Qualifications
  • Active, unencumbered LPN/LVN license in the state of practice
  • Current CPR certification (with hands‑on component)
  • TB skin test current within last 12 months
  • Six months prior hands‑on nursing experience preferred but not required
  • Reliable transportation
Essential Job Functions
  • Provide, coordinate, and document skilled patient care in accordance with physician orders and care plans
  • Continuously observe and assess patient condition and care needs, reporting changes to supervisor or physician as appropriate
  • Document all activities, assessments, nursing actions, responses, and care coordination promptly
  • Participate, implement, and update the nursing care plan
  • Take appropriate nursing actions based on assessment to achieve expected outcomes
  • Recognize changes requiring intervention and implement care to prevent or reduce risk
  • Comply with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act
  • Provide patient care using infection control measures that protect staff and patients per OSHA standards
  • Educate patient and family on disease process, self‑care techniques, prevention strategies, and meet patient nursing needs
  • Maintain competency by participating in educational programs, continuing education units, internal learning, and skill evaluations
Requirements
  • Graduate of an accredited school of nursing
  • Current, unrestricted state LPN license
  • Current CPR certification
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures
Additional State‑Specific Requirements
  • South Carolina – ≥1 year of pediatrics experience
  • California – ≥1 year of experience under current nursing license
  • Louisiana – ≥1 year of experience as a licensed nurse
  • Continuing education as required by state
Preferences
  • Six (6) months recent experience as a licensed nurse in a clinical care setting
  • Home health experience

  • Maintain visual and hearing acuity and a strong sense of smell and touch
  • Respond physically and mentally to emergencies in home or transport
