What You'll Do
- Provide direct patient care, including monitoring vital signs, performing assessments within LPN scope, and reporting changes in patient condition to the RN and healthcare team.
- Administer medications, treatments, and wound care as prescribed while ensuring accurate documentation and patient safety.
- Assist patients with activities of daily living (ADLs), such as bathing, dressing, mobility, and toileting, while promoting independence whenever possible.
- Collaborate with registered nurses, physicians, therapists, and other healthcare professionals to implement and support individualized patient care plans.
- Educate patients and families on medications, discharge instructions, disease processes, and recovery expectations while reinforcing teaching provided by the RN and care team.
What We're Looking For
- Education:
High School Diploma or GED, required. Graduate of accredited Practical Nursing Program, required. - Experience:
One year of relevant experience, preferred. - Licensure/Certification:
Appropriate state LPN license, required. - Additional
Skills Required:
Computer skills, phone call etiquette, organizational skills.
