Responsibilities- Conduct/participate in quarterly quality improvement meetings, reviewing data and implementing improvement plans.
- Collaborate with Branch Director to execute quality improvement activities and corrective action plans.
- Oversee responses to external and internal surveys and audits within the assigned area related to quality of care and clinical KPIs, in collaboration with operations leadership.
- Complete documentation and chart reviews to identify variances in standardized care processes and improve patient care.
- Hold case conferences to ensure oversight of care coordination and standards.
- Facilitate the development of performance improvement action plans, communicate job expectations, plan, monitor, and evaluate job results, coach, counsel, and discipline employees; develop, coordinate, and enforce systems, policies, procedures, and productivity standards.
- Ensure staff evaluation is fair and timely per company procedures.
- Bachelor’s Degree preferred in Nursing from an accredited school.
- Licensed Registered Nurse (RN) in good standing, licensed in the state of practice and residence.
- Less than two years of clinical experience, at least one year in a related management/supervisory role in home health.
- Current CPR certification.
- Meet all agency pre-employment requirements per company or State regulations.
- Proficient with company documentation systems.
- Strong communication skills (verbal and written) with all personnel levels.
- Ability to work independently and as part of a team.
- Capable of handling confidential information responsibly.
