Summary
The Registered Nurse (Patient Transfer Coordinator) is accountable for patient care through the coordination of admissions, transfers of Veterans to and within the Veterans Affairs Medical Center (VAMC), along with those originating from other facilities both Veterans Affairs (VA) and non- VA.

The Registered Nurse, Patient Transfer Coordinator (PTC) is responsible for providing competent, evidence-based care coordination to assigned patients as appropriate to the setting. The Patient Transfer Coordinator practices according to guidelines established by VISN 6 and facility leadership for internal and external transfer to Veteran patients. The PTC Registered Nurse is an active member of an interdisciplinary team whose primary focus is on care transitions to and from an inpatient setting.
Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Follows procedures per established policies and guidelines. Other related duties include, but are not limited to the following:
Coordinates and collaborates with other VA and non-VA departments, community agencies, and healthcare providers to ensure patient needs are addressed. Applies the nursing process which includes systematic data gathering, assessment, and appropriate nursing judgment to ensure positive patient outcomes. Provides indirect nursing care to patients with complex needs as related to transfers, admissions, telephone triage, and discharges. Collaborates with the interdisciplinary team to validate and/or collect initial patient information such as Veteran benefit eligibility, insurance status, Veteran travel eligibility, facilitates physician to physician communication, nurse to nurse handoff, transportation needs and bed availability at the VAMC.
Assists with patient planning assessments, identification of barriers, and activities required to successfully transition the patient in and out of the acute inpatient setting. Facilitates the transfer process by working with the interdisciplinary team to establish appropriateness of admission or transfer-out based on review of medical information. Performs day-to-day case management activities and discharge/transfer planning of Veterans in Non- VA Facilities by maintaining appropriate and constructive relationships with all clinical and non- clinical staff involved in the Veteran's care.
Contributes professional nursing perspective in discussions with the interdisciplinary team. Performs other related duties as listed in functional statement. VA offers a comprehensive total rewards package: VA Nurse Total Rewards Pay:
