Service Coordinator - North Carolina

GENERAL PURPOSE

Responsible for service management function within assigned facilities. This function includes development of contacts with service providers and agencies for resident referrals and ongoing service management responsibilities. Educates residents on available services and monitors provisions of services. Works in conjunction with management staff of the facility.

• Assist with assessing applications, assigning housing units, maintaining wait list for each housing complex, processing tenant requests and resolving tenant problems/concerns, and assisting with annual recertification’s of tenants. Also, assist with processing rents, counting receipts and making deposits.
• Assists and advises residents and families of the services which may be necessary to maintain a self-reliant lifestyle. Schedule Health Screening programs, facilitate educational events and seminars relating to health care to include ensuring Federal/State/County support information for health and financial needs are made available to tenants.
• Educates residents, families and staff on available community resources, as well as service availability, application procedures, resident rights, etc., both individually and as a group.
• Works as a team member with housing managers by following up on resident situations the managers believe may need supportive services. Refer to the Council on Aging and other social service agencies for Meals on Wheels, Housekeeping, Hospice, etc.
• Facilitates meeting of needs when necessary but avoids the creation of unhealthy dependence. Uses the least drastic intervention necessary to alleviate a problem situation.
• May assist residents or coordinate training for residents in understanding lease and tenancy obligations.
• Monitors the delivery of services to residents to ensure they are appropriate, timely and satisfactory. May also manage such services when appropriate, to include arranging transportation for medical and shopping needs of tenants. Maintain communication and transportation logs. Create service management plan as appropriate.
• Provided limited case management (i.e., evaluation of social, psychological and physical needs and the development of a service plan) for a resident when such service is not being provided. Pursue avenues for services through private, local , state and federal sources.
• Document contact with residents, providers and families. Resident files are to be kept in a secure area to ensure confidentiality.
• Maintain individual files on residents which contain at least: intake information, service initiation and termination, quarterly review and follow-up, human or civil rights abuse, resident/family meeting notes.
• Provide on-call 24/7 services on a back-up rotational basis for Housing emergencies.
• Follow all policies and procedures of the Osceola Council on Aging, as well as any applicable Housing guidelines.
• Other duties as assigned to include assisting in times of disaster as needed.

HOURS OF WORK:
Full-time 40 hours/weekly – office open 8:00 am to 5:00 pm, flex time with supervisor’s approval. May require some evening or weekend hours for special projects. Additionally, the employee must be able to work off-site at any of the locations in the Planning and Service Area.

High School diploma or GED. Ability to read and write, carry out written and verbal instructions and complete required reports. Aware of eligibility for and procedures of federal and state programs. Demonstrated working knowledge of community services in the region with particular knowledge of services that are provided for the population living within the facility.

One to two years proven experience in service management, including organizing, problem solving and advocating. Alert to legal liability issues relating to providing service coordination. Must have a valid Florida driver’s license and reliable vehicle.

Must have excellent interpersonal, communication and organizational skills and the ability to work with a wide variety of groups, individuals and cultures.

Sufficient skills to be able to communicate clearly and effectively, problem solve, and be highly organized while moving throughout multiple properties as required.

Knowledgeable and competent to perform all essential functions.

Ability to visually and audibly perform all essential functions.

Must be able to sit for long periods of time. Sufficient mobility and strength to move about the facility and the community, have corrected vision and hearing to the normal range. This position may require occasional bending and lifting subject to specific physical requirements and working conditions as listed in job duties.
