We are looking for a Utah licensed Clinical Pharmacist to join a strong team to utilize Arine s software platform and help optimize patient outcomes for our clients. The role provides expert level medication therapy consultation to patients and providers, collaborates with the Engineering team to improve Arine s software, and helps supervise the Clinical Care support team. The candidate will impact patient care, stay up-to-date on developments in pharmacotherapy, and be aware of emerging patterns, guidelines, and best practices.

The Clinical Pharmacist operates under the supervision of the Head of Operations to carry out and supervise direct patient care responsibilities, including maintenance and review of patient medication profiles; counseling patients on medication use; promoting adherence; developing clinical recommendations; reviewing completed medication reviews; and collaborating with other health professionals to improve patient outcomes. Responsibilities include orienting new team members, facilitating prior authorizations, helping patients access financial assistance and transportation to support adherence, and evaluating clinical guidelines and workflow processes.

All members of the Clinical Care team are expected to communicate effectively, maintain cooperative relationships with clients and colleagues, be respectful, accept constructive feedback, and continuously learn new skills to deliver quality service and patient care.

• Evaluate, assess, and document patient data in Arine s software platform and other programs; prepare written and oral reports on patient care and team progress
• Complete efficient and thorough medication reviews and/or adherence counseling
• Review completed medication reviews and counseling by other Virtual Pharmacist users, as assigned
• Staff various clinical shifts and work with automation and new technology
• Respond to questions from patients, providers, and other professional personnel and team members
• Participate in process improvement meetings to optimize workflows
• Communicate effectively in written and oral formats and provide daily sign-out when applicable
• Follow up with clinics, providers, pharmacies, and patients to resolve issues promptly
• Develop intervention strategies to optimize medication utilization and achieve quality measures
• Identify opportunities to improve workflow processes and Arine s software platform
• Assist with obtaining referrals and authorizations
• Guide and help oversee other Clinical Care team members
• Orient new Virtual Pharmacist users to workflows and the Virtual Pharmacist
• Evaluate and help develop clinical algorithms
• Comply with all company policies and state and federal rules and regulations

• Doctor of Pharmacy (Pharm
  
  D) from an accredited college/university
• Utah Licensed Pharmacist in good standing (multiple state licenses a plus)
• Completion of a PGY-1 residency, or equivalent experience
• Ability to review and assess prescription appropriateness with age-specific and disease-specific considerations
• Ability to engage with patients, promote adherence, and treat them with empathy
• Ability to organize and set priorities reflecting job responsibilities
• Knowledge of chronic disease states such as diabetes, heart failure, COPD, asthma, cardiovascular disease, hypertension, dyslipidemia, anxiety, depression, bipolar disorders, and thought disorders
• Ability to apply judgment to work independently and meet deadlines
• Ability to work effectively within a team and foster relationships
• Experience working with patients from diverse backgrounds
• Knowledge of factors affecting drug activity and disposition
• Knowledge of regulatory requirements related to areas of responsibility
• Knowledge of prescription and non-prescription drugs and their indications, efficacy, dosage, and contraindications
• Willingness to obtain registered pharmacist licenses in other states as needed
