As a vital member of the Health and Housing Transition (HHoT) Team at Fourth Street Clinic, the Medical Case Manager plays a key role in transforming lives for those within Palmer Court Apartments. This dynamic position involves assisting patients and families in navigating the maze of medical, social, educational, and other supportive services necessary for their well‑being. Located in a permanent supportive housing program for individuals and families with a history of chronic homelessness, you'll be at the forefront of making meaningful connections.

With collaboration as your superpower, you'll work closely with tenants and community partners like The Road Home, managing referrals and follow‑ups for healthcare services, insurance eligibility, transportation, and addressing social determinants of health. A high school diploma or equivalent is required, and having a background in social services or community work will set you up for success. You'll be expected to obtain your Utah case management certificate within 90 days of your hire, so a commitment to professional growth is key.

Preferred qualifications include a bachelor's degree in social work or a related field, along with a Social Service Worker (SSW) Utah license. Experience in medical case management or care coordination, particularly with homeless or vulnerable populations, will enhance your ability to make a significant impact.
