Position Summary

The ACT Nurse provides essential medical and nursing support to individuals receiving services through Assertive Community Treatment (ACT) programs. This role is responsible for coordinating medical care, managing medications and medical supplies, performing clinical nursing functions, and serving as a liaison between behavioral health teams and medical providers. The ACT Nurse supports up to two ACT teams and works collaboratively within a multidisciplinary treatment model to ensure safe, effective, and integrated client care in community-based settings.

Schedule: Monday - Friday 8/9 am - 4/5 pm

• Manage and coordinate new client admissions, including assessment and facilitation of immediate medical needs such as urgent medications, medical appointments, and dental care.
• Manage weekly medication inventorying and ordering for up to 200 psychiatric clients living in the community.
• Provide basic first aid and wound care; triage acute medical and psychiatric situations and respond appropriately.
• Administer intramuscular (IM) injections in accordance with clinical standards and agency protocols.
• Assist walk-in clients with medication management services.
• Communicate with internal and external clinics, hospitals, medical practices, and pharmacies to coordinate medications, medical records, continuity of care, and referrals.
• Serve as an effective liaison between behavioral health programs and medical providers, clearly communicating medical and behavioral health plans to multidisciplinary teams.
• Oversee purchasing, inventory management, storage, and distribution of medical supplies within ACT program offices.
• Audit controlled substances daily in compliance with regulatory and agency requirements.
• Accurately document all client interactions, medication services, and clinical activities in the electronic health record (EHR).
• Document incidents and complete required reports in a timely and compliant manner.
• Conduct quarterly walk-throughs of relevant accreditation and regulatory standards in ACT program medical offices.
• Participate in daily multidisciplinary team meetings, contributing clinical insight and care coordination support.
• Participate in the agency’s on-call rotation as required.
• Serve two separate ACT teams, with time divided between two offices; assignments are subject to change based on agency needs.
• Perform other duties as assigned to support program operations and client care.

• Current, unrestricted licensure as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the State of Utah, in good standing.
• Eligibility to work in the United States.
