Position Overview
The Triage Nurse at Fourth Street Clinic promotes and restores patient health by collaborating with physicians and a multidisciplinary team in the primary care setting. This role provides both physical and psychological support to patients while managing patient flow through the clinic via triage. Triage Nurses play a critical role in assessing, prioritizing, and coordinating care for patients seeking same-day services, ensuring high-quality care for all.
This is a full-time, 40-hour per week position
, with occasional fluctuations in daily hours as needed. Triage Nurses may perform all nursing responsibilities when covering the clinic and are expected to complete additional duties as assigned.
Patient Care & Triage
- Quickly and efficiently triage all patients seeking same-day services.
- Organize and manage provider schedules based on medical and behavioral health acuity.
- Maintain sensitivity and cultural competence when working with unsheltered and diverse populations.
- Answer the triage phone during clinic hours to coordinate with hospitals and community partners.
- Develop individualized patient education plans, including learning objectives and teaching methods.
- Evaluate the effectiveness of chronic disease management plans with patients and providers.
- Participate in quality improvement initiatives to improve patient outcomes.
- Ensure accurate and timely documentation of patient care in the electronic health record.
- Report safety concerns to supervisors immediately.
Administrative Duties
- Maintain general office skills, including phone etiquette, computer proficiency, and filing.
- Assist with special projects, including preventative health programs and UDS (Universal Data Set) initiatives (mammograms, FIT testing, Pap clinics).
- Support clinic quality improvement initiatives in collaboration with staff.
- Implement new processes and administrative projects as assigned by the Nursing Director.
- Ensure smooth daily clinic operations and timely resolution of needs.
- Communicate departmental activities, changes in policies or procedures, and other relevant information to staff and committees.
- Complete incident reports as outlined by clinic policy.
Leadership & Teamwork
- Demonstrate interpersonal skills to support teamwork and a positive clinic environment.
- Communicate effectively with staff, providers, volunteers, patients, and community organizations.
- Address minor problems proactively, escalating as appropriate.
- Model honesty, fairness, and consistency in all interactions.
- Support team members, mentor peers, and accept feedback constructively.
- Maintain understanding of clinic policies and procedures to enhance staff productivity and satisfaction.
Licensure & Compliance
- Maintain an active Registered Nurse license in the State of Utah
. - Work within the scope of nursing licensure and seek consultation from providers as needed.
- Facilitate RN visits across all FSC sites as required.
