The Assertive Community Treatment (ACT) Team provides intensive, community‑based services to adults living with serious mental illness, including:
- Schizophrenia
- Schizoaffective Disorder
- Bipolar Disorder
- Major Depression with Psychotic Features
- Co‑occurring Substance Use Disorders
Our team is dedicated to helping individuals achieve stability, independence, improved health outcomes, and long‑term recovery.
What You’ll DoAs the ACT Team Registered Nurse, you will serve as a vital connection between psychiatric and physical healthcare. You’ll help clients manage medications, monitor overall health, coordinate medical care, and ensure that physical health concerns receive the same level of attention as mental health needs.
Key Responsibilities- Provide nursing services in community‑based settings, including client homes, shelters, and other locations
- Administer medications, including oral medications and long‑acting injectable antipsychotics (LAIs)
- Monitor medication effectiveness, side effects, and adherence
- Conduct nursing assessments, health screenings, and wellness checks
- Coordinate laboratory monitoring and communicate results to the treatment team
- Collaborate closely with the psychiatrist/prescriber regarding medication management and treatment planning
- Coordinate care with primary care providers, specialists, hospitals, and community healthcare partners
- Provide education regarding medications, chronic disease management, nutrition, wellness, and preventive healthcare
- Participate in daily interdisciplinary team meetings and integrated treatment planning
- Assist with crisis intervention and provide nursing consultation during on‑call rotations
- Maintain accurate and timely documentation in the Electronic Medical Record
- Build meaningful relationships with clients and support their recovery journey
- Work within a highly collaborative multidisciplinary team
- Practice nursing in a dynamic and engaging community‑based setting
- Help reduce barriers to healthcare for vulnerable populations
- Make a direct impact on both mental and physical health outcomes

- Current, unrestricted RN license in the State of Utah
- Bachelor’s Degree in Nursing (BSN) preferred;
Associate Degree in Nursing (ADN) with relevant experience may be considered - CPR/BLS Certification
- Valid driver’s license and insurability
- Ability to travel throughout the community to provide services
- Two or more years of nursing experience
- Experience in psychiatric nursing, community mental health, behavioral health, inpatient psychiatry, or crisis services
- Experience administering long‑acting injectable (LAI) medications
- Experience working with individuals experiencing co‑occurring mental health and substance use disorders
- Familiarity with Medication‑Assisted Treatment (MAT)
- Psychiatric Mental Health Nursing Certification (RN‑BC)
- Recovery‑oriented and trauma‑informed
- Passionate about serving individuals with serious mental illness
- Skilled at building trust and engaging clients in care
- Comfortable working independently in community settings
- Collaborative and team‑focused
- Calm, flexible, and confident in crisis situations
- Committed to treating clients with dignity, respect, and compassion
