Registered Nurse (RN) – Community Mental Health Job Details

  • Location: Salt Lake County, UT (On-site / Community-Based)
  • Schedule: Monday – Friday, Day shift with rotating on-call evening coverage (Medical Sub-Team rotation Friday–Sunday evenings)

About the Role

We are seeking an autonomous, clinically strong Community Mental Health Registered Nurse (RN) to provide critical, community-based psychiatric nursing care for adults living with serious mental illness. Working as a fully integrated member of a multidisciplinary team, you will act as the vital bridge between psychiatric and medical care.

Because this role operates directly out of the community rather than a traditional clinic setting, you will have the clinical autonomy to operate independently in unpredictable environments, ensuring that our clients' physical health receives the same meticulous attention as their psychosocial and psychiatric needs.

Key Responsibilities Clinical & Client Services
  • Community Medication Management: Administer psychotropic medications to clients in community settings, including directly observed oral therapies and long-acting injectable antipsychotics (LAIs).
  • Health Monitoring & Assessment: Conduct independent nursing assessments (vital signs, physical health screenings, and psychiatric symptom observations) directly in the field.
  • Laboratory Coordination: Manage laboratory monitoring protocols—ordering, tracking, and communicating results for metabolic panels, clozapine monitoring, and lithium levels in coordination with the Psychiatrist.
  • Continuity of Care: Coordinate with primary care physicians, specialists, emergency departments, and community health partners to ensure seamless physical healthcare integration.
  • Client

    Education:

    Partner with clients to provide guidance on chronic disease self-management, medication side effects, nutrition, and preventative care.
Team Collaboration & Administration
  • Daily Interdisciplinary Meetings: Participate in daily team meetings, delivering healthcare status updates and contributing to comprehensive, shared treatment planning.
  • Informal Leadership: Provide medical consultation, health monitoring advice, and safety protocol guidance to the broader interdisciplinary team.
  • Compliance & Documentation: Maintain detailed, compliant documentation of all nursing contacts and health assessments in the Electronic Medical Record (EMR) according to county, provider fidelity, and agency standards.
Qualifications & Requirements Education & Experience
  • Degree:
    Bachelor’s Degree in Nursing (BSN) from an accredited program is required. (An ADN with equivalent high-level clinical experience may be considered).
  • Experience:

    Minimum of two (2) years of clinical nursing experience. Preference is given to backgrounds in psychiatric nursing, community mental health, inpatient psych, or crisis services.
  • Preferred

    Skills:

    Experience administering long-acting injectables (LAIs), working with co-occurring substance use disorders, and familiarity with Medication-Assisted Treatment (MAT) protocols.
Licenses & Certifications
  • Current, unrestricted Registered Nurse (RN) license in the State of Utah (Required).
  • Psychiatric-Mental Health Nursing Certification (RN-BC) through the ANCC (Strongly Preferred).
  • Active CPR / BLS Certification (Required).
  • Valid Driver’s License.
