Job Summary

The Department of Pediatrics at the University of Utah School of Medicine is seeking a Clinical Nurse Coordinator to function as a member of the Pediatric Gastroenterology team at Primary Children’s Hospital. The team consists of 10 RN’s working with 4 NP’s and 21 MD’s. Responsible for delivering professional nursing care to patients in-person, via phone, or via patient portal. Utilizes strong organizational and leadership skills to assess patient status;

implements clinical nursing interventions to modify symptoms; develops, implements, and evaluates patient care plans and patient education materials to enhance and improve outcomes; and ensures a safe and therapeutic environment.

This position requires an on-site presence most days per week, but may allow for occasional telecommuting work, dependent on operational needs. The on-site days will vary each week depending on the assigned providers’ schedules. Job location is expected to be primarily at the Intermountain Primary Children’s Hospital Salt Lake City campus, but may also require a few days per month at the Intermountain Primary Children’s Hospital Miller Campus located in Lehi, UT or Layton, UT depending on clinical needs.

Responsibilities
  • Performs phone management with patients’ parents, chart review, care coordination, triage, and problem management.
  • Discusses patient and family concerns in person/over the phone and/or via the electronic health record.
  • Provides and develops patient/family education materials surrounding common Gastroenterology disorders; communicates education best practices to proactively answer common questions to reduce future messages.
  • Communicates adjustments in treatment plan to families.
  • Formulates care plans for specific patients and documents the nursing process in patient’s record.
  • Evaluates patient response to interventions and modifies plan of care as needed to achieve health care goals. Communicates adjustments or changes in treatment plans to patient/family.
  • Develops and participates in educating patients and their families about care programs.
  • Communicates lab & other test results to families.
  • Provides medication refills.
  • Assists providers and staff with patient-related paperwork such as letters of medical necessity, DMV forms, school care plans, and preauthorization.
  • Maintains a safe and therapeutic patient care environment.
  • Works closely with triage team and gastroenterology staff to provide high quality, efficient care to patients and their families.
  • Keeps current on advances in nursing care by participating in educational programs.
  • Orients and educates new staff in nursing techniques and departmental procedures.
  • Utilizes knowledge of medications, procedures, infection control, and safety precaution, and initiates appropriate interventions in crisis situations.
  • Participates in total quality management to positively affect patient and system outcomes to improve care.
  • Provides nursing support to the pancreatitis clinic, managing care coordination, triage, patient education, and follow-up for a complex pediatric pancreatitis population.
  • Coordinates closely with pancreatitis‑specialized physicians, the Advanced Endoscopy Clinical Nurse Coordinator, registered dietitians, behavioral health providers, and other interdisciplinary team members to support comprehensive, coordinated patient care across the continuum.
  • Prepares patients for clinic visits by completing thorough chart review and pre‑visit planning, including summarizing interval history since the last visit (e.g., hospitalizations, emergency department visits, laboratory results, imaging, procedures, medication changes, and external records) to support efficient clinic flow and accurate provider documentation.
